4. Respite Care*
5. Substitute Family Care (SFC)

An Adult Day Care Center is a program that acts primarily as training to help individuals with intellectual disabilities to reach their highest practicable functional level. Adult Day Care is designed to meet the individual needs of functionally impaired adults. This is a structured and comprehensive group program which provides a variety of health, social, and related support services in a protective setting for a portion of the 24-hour day. This program differs from an Adult Day Health Care Program which is a medical model, designed to provide services for medical, nursing, social, and personal care needs to adults who have physical, mental or functional impairments. Such services are rendered by utilizing licensed professionals in a community based nursing center.
A person, or provider, who desires licensure to provide one or more of the above services must first read & understand information in the regulations. Modules that have an asterisk (*) require approval from the Facility Need Review committee before the provider can apply for a license from Health Standards Section.
Persons who desire to open a Home and Community Based Service must attend a state sponsored training class BEFORE a licensing survey will be scheduled. A licensing survey is one of the requirements for initial licensure.
