These guides describes the community resources that care coordinators working with families with children and/or youth with special health care needs might find helpful. The information listed helps professionals identify who the program serves, how to contact the program, if the program has eligibility criteria, and also if the family needs to have a Medicaid Community Care Referral and a physician referral.
Levels of Care (LOC) are assigned to all patients that qualify as a Medical Home patient. This means the patient must be “linked” to the practice with the practice being the “primary care” physician.
