Welcome to the Commodity Supplemental Food Program (CSFP) Participant Preliminary Certification Webpage

Thank you for your interest in becoming a participant of the Commodity Supplemental Food Program (CSFP). The CSFP program works to improve the health of income eligible participants by offering nutritious USDA commodity foods.

The types of individuals that CSFP serves include the following:

  • pregnant women
  • breastfeeding women,
  • postpartum women up to one year after delivery,
  • infants
  • children up to their sixth birthday
  • seniors at least 60 years of age

How Do I Qualify for CSFP?

To participate in the CSFP program, applicants must reside in the state of Louisiana and meet CSFP income eligibility requirements established by the Federal Poverty Income Guidelines.

Important! Individuals who are currently enrolled in the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) do not qualify to participate in the CSFP program.

How Can I Apply for CSFP? 

If you feel that you might qualify for the CSFP Program and would like to apply, please follow these steps.

       1.  Access, fill out and print the Preliminary Certification Form for CSFP. 

       2. Mail the completed application to the following address:

       3. Or you may fax your completed application to 504-241-3102

 

 I've Sent in My Form. What Now?

Once your CSFP Preliminary Certification Form has been received, the Food for Families organization will review your information to determine your eligibility. You will be notified within 10 business days of receipt of your completed form on whether you are eligible for the program.

You will also receive information on how to reach a local CSFP distribution center to complete your certification if you are eligible. You will need to bring additional information with you to finish signing up for CSFP. Find out what additional information you will need here.

Please note that placement on the program is dependent on availability of caseload space or slots. When slots are unavailable, you may be placed on a waiting list and later certified as slots become available on a first-come, first-served basis. You will be notified within 10 business days of receipt of your completed form if you are to be put on a waiting list for the program.
